PROCEDURE OF DENTAL IMPLANT PLACEMENT

The dental implants are inserted into the bone of the jaws. The area is first made numb by injecting local anaesthesia. Sinus Lift

The roots of our upper teeth mainly the back ones are in very close vicinity of Maxillary Sinus. Sometimes when the teeth are extracted prematurely because of any reason and are not then replaced for quite some time, this Maxillary sinus tends to shrink away because there is no tooth or a root to hold that empty cavity. In such cases it is not possible to place implants because of lack of sufficient bone height.
The procedure used to increase or augment the height of the bone in that area is called as sinus lift. It can be either a direct (open )sinus lift procedure or an indirect (closed ) sinus lift procedure.

Indications for sinus ift procedure are where bone may be less because of the following factors.

  • 1.Small jaw size and large sinus
  • 2.Gum disease
  • 3.Bone resorption due to long standing tooth removal
  • 4.Bone loss due to surgical tooth removal or the presence of an anamolus condition like a tumour or a cyst.

INDIRECT / CLOSED SINUS LIFT PROCEDURE

This is relatively simple technique. Here the osteotomy site is first drilled upto the available bone height. Then special surgical instruments called osteotomes are placed inside the osteotomy site in increasing width and length and then slowly tapped to lift the sinus membrane, bone graft is then placed and an increase in the available bone height is achieved. The minimum height of the bone required for an indirect sinus lift procedure to be performed is 4-5 mm.

Sequential steps in indirect sinus augmentation technique

Direct / Open sinus lift procedure

In this procedure, a window ismade surgically in the area of the upper jaw or the maxillary bone where the implant needs to be placed. After the window is drilled surgically,and the bone plate removed from that area, one can see the lining and wall of the sinus. Block bone grafting technique

This technique is used in severely atrophic lower back jaw where the implants need to be placed in the area of first and second molars. This technique is used to reposition inferior alveolar nerve. Ridge Modification
